All Vitalis HealthCare forms in one place — report an incident, an illness, a schedule problem, a complaint, or a leave request, ask about care, or apply for a job.

Our forms have moved. They now live in one place, so you no longer have to work out which link to use before you start.

The page is called Tell the office, and it asks you one question: what would you like to tell us? Choose the answer that fits your situation and it will take you to the right form. There is no password to remember and nothing to install, and every form is designed to work properly on a phone.

Please use the new page rather than an older form link you may have saved or bookmarked. Some of the old links still open, but what you send through them no longer reaches the office the same way.

If someone is in danger or needs urgent medical help, call 911 first. Fill in the form afterward, once everybody is safe.


Pick the one that fits

Something happened to a client or caregiver

Use this for a fall, an injury, a hospital visit, a near miss, or anything else that worried you. Tell us as soon as you can, even if it turned out fine in the end — a near miss today is often how we prevent something serious later.

Someone is unwell

Use this whether the person who is unwell is a client, another staff member, or you. It covers an infection, a fever, or any symptoms you believe the office should know about, including symptoms that seem minor at the time.

A problem with a visit or the schedule

Use this for a missed visit, arriving late, leaving early, a canceled visit, or a client who did not want care that day. Tell us what happened even when the client was fine about it.

Time off, a missed shift, or leaving Vitalis

Use this if you cannot make a shift, you would like to request time off, or you are giving your notice. Tell us as early as you are able to, because the sooner we know, the more time we have to arrange cover for your clients.

A concern or complaint

Use this when something has gone wrong and you want it looked into properly. Clients, family members, and staff can all use this form, and every complaint we receive is reviewed.

An update for the office

Use this for anything the office should know that is not a problem — a change of address or phone number, a note about how a client is doing, or anything else worth passing on.

What happens after you send a form

Your report goes straight to the office, and the right supervisor is notified automatically based on what you reported and which client it concerns.

When you finish, a reference number appears on the screen — something like INC-2026-0001. Write it down or take a screenshot of it. If you call us about that report later, the number lets us find it immediately.

If a situation is urgent, please do not wait for a reply to your form. Call us.
