COVID-19 Self-Screening

COVID-19 Symptoms:

  • Fever or chills *
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

Consider Your Exposure Risk:

  • Have you had contact with anyone with a suspected or confirmed case of COVID-19 in the last two weeks?
  • Have you attended any indoor gatherings of 10 or more people?
  • While moving through public spaces, have you worn a mask at all times?
  • After being in public spaces, have you washed your hands thoroughly?
  • To check your temperature accurately, you need to:
    • Not drink or eat right before checking your temperature
    • Make sure that the thermometer is in place under your tongue, as far back as possible
    • Take your temperature twice (this gives the thermometer time to come up to body temperature)

Being aware of symptoms and exposure risks is crucial to preventing the spread of COVID-19. All staff should monitor their temperature (using the method above) and communicated any suspected COVID-19 infection to the Director of Nursing.
